New Patient Registration

building.jpgIf you live within our practice area please ask at reception or ring 01942 483330 for an appointment to register. The receptionist will give you some forms that need completing, you need to bring them with you to the appointment.

You also need to bring a sample of urine (pot will be provided) and proof of identity.

All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.

Medical treatment is available from the date of registration. Please contact reception for further information.

Changes to Personal Details

Please let us know if you change your name, address or telephone number.

New babies should be registered with a doctor as soon after leaving hospital as possible.
